Pennsylvania Service Centers

The State of Pennsylvania requires that ignition interlock installers' three year driving history and criminal record be clear of any alcohol related incidents.  CST will preform this checks on any new installer and yearly on existing installers as required by Pennsylvania Law.  Please download the attached form and submit it along with your Service Center Agreement for approval.
