If you are interested in becoming an Authorized Intoxalock^® Service Center, download the Service Center Agreement. Fax the completed form along with a copy of your insurance binder to 515-331-7513.

If you are located in the following states, please download the accompanying state forms and submit completed copies with the Service Center Agreement for approval.
